Louisiana Public Service Commission ("LPSC"
or "Commission") General Order 2 dated July 1, 1921
and General Order dated March 15, 1996 (as amended December
10, 1999, In re: Regulations for Competition in the Local
Telecommunications Market) require all companies providing
utility and telecommunications services within the state of
Louisiana and under the jurisdiction of the LPSC to file an
annual report of its financial and operating conditions.
The filing deadline for annual reports is 90 days
after the end of the fiscal year. Effective January
1, 2001, the penalty for failure to file an annual report
by the filing deadline is $5,000 for the first offense and
up to $50,000 for offenses thereafter. Repeated offenses
or failure to pay fines may result in the revocation of a
company's certificate of authority. All companies
are expected to print the appropriate forms, complete the
forms, and return the forms to the Commission on or before
the filing deadline. The return address is:

In addition, all companies are required
to immediately notify the Commission, in writing, of their
fiscal year end. Any future changes to the fiscal year
end should also be provided to the Commission in writing.

be received by the Commission two weeks prior to the filing
